Case Management
Through long- and short-term case management services, the Network links informal and formal support systems into a plan of coordinated care for frail elders. Case managers conduct home visits and return as often as necessary to establish rapport, build trust and determine clients' strengths and needs. They develop a coordinated care plan, determine entitlement eligibility, interpret forms and instructions, act as liaison/advocate and arrange or provide transportation. The case management program serves over 600 seniors annually.
Helping Hands at Home
Helping Hands at Home” is a program designed to employ Bayview Hunters Point residents as attendant care workers in the homes of low-income frail elders in the community. The Network has home care workers on staff who provide in-home care to especially needy case management clients. The clients pay the Network $6-$10.00/hour, depending on their ability to pay. With assistance from grants and donations, the Network supplements the fees so that the workers receive fair wages, basic benefits, holiday, sick and vacation pay.
Project VOICE
Project VOICE (Volunteers Offering Information and Companionship to Elders) is an interfaith volunteer caregiver program. Local churches and youth programs partner with Project VOICE to develop a leadership group that provides a framework for volunteer services for Bayview Hunters Point seniors. The group has elected its own leaders, including representatives from each church involved. The program currently offers phone reassurance, yard work/minor home repairs, friendly visiting, respite assistance, grocery shopping, and transportation for Network clients. The Project VOICE director also oversees the monthly Network membership meetings and helps members plan fun activities for the seniors.
Seniors in Motion for Health
The Network’s Wellness Program provides information to Network members and clients on health issues, particularly those affecting low-income elderly. We offer workshops, classes, and town hall meetings on health and environmental issues. Our newest project—Seniors in Motion for Health—is an exercise promotion program offered in collaboration with UCSF and the California Endowment. We are now providing two exercise classes per week as well as weekly workshops on nutrition, disease and pain management, proper exercise, health maintenance and disease prevention. Approximately 50 people are involved in the classes and workshops at this time. We hope to increase participation, as the project becomes a fully integrated and permanent offering.
